Cancellation Policy: There is a $120 cancellation fee for any cancellation made within 24 hours of a scheduled appointment.
Paperwork Fee (when occurring outside of the session and other than sent to healthcare provider for care or treatment): $250 per request, which includes postage and shipping.
Court Fee: A retainer of $3500 is required at a rate of $650 an hour for local court (Maricopa County, AZ).
